  • Anti-Hazing Statement: Chapter President signs and submits the FSA Anti-Hazing statement indicating that they have received a copy of the hazing policy, they have had an opportunity to ask questions, and they will educate their members about the policy.
  • Certified Housing Workshop: Undergraduate live-in member attends the Certified Housing Workshop each semester.
  • Chapter Advisor: Chapter has a chapter advisor and has provided contact information to Fraternity & Sorority Affairs.
  • Chapter Officer Updates: Current chapter officers are indicated on chapter roster and in contacts section.
  • Chapter President Leadership Academy: Chapter President is to attend the Chapter President Leadership Academy.
  • Chapter President Meeting: Chapter President meets with chapter's FSA staff liaison at least once per semester.
  • Chapter Semester Report: Chapter President submits the Chapter Semester Report.
  • Fire Safety Online Module:
    One member completes online fire safety module
    • Chapters with Certified Housing status: House Manager completes module
    • Chapters without Certified Housing status: Chapter president can select a general member to complete this module
  • GUARD:
    • Chapter must meet attendance requirements for GUARD: Consent & Sexual Assault Prevention for Fraternities & Sororities Workshop
    • For every 50 members a chapter had on their spring roster, the chapter must send one representative during the academic year.
      • 0-30 members: 1 participant
      • 31-60 members: 2 participants
      • 61-90 members: 3 participants
      • 91-120 members: 4 participants
      • 121-150 members: 5 participants
      • 151-180 members: 6 participants
  • Hazing Prevention Program: One member (Preferably New Member Educator) attends an FSA sponsored hazing prevention program each calendar year.
  • Member of a Council: Chapter is a member of either Black Greek Council, Interfraternity Council, Panhellenic Council, or United Greek Council.
  • New Chapter President Orientation: Newly elected chapter president completes the New Chapter President Orientation module in Canvas their first semester in office.
  • New Member Rights and Grade Release Card: Facilitate completion and submission* of New Member Rights & Grade Release Forms. It is an expectation for each and every new member who joins the fraternity and sorority community at Illinois to complete and have on file in our office a New Member Rights & Grade Release Form. While our office has an expectation for each members to sign and date the "Guaranteed Rights of Members," we encourage chapter leaders to have member sign and date the Fraternity & Sorority Grade Release Form in order for our office to list each member's grades on the academic grade report.

    * All members must be added to the chapter roster to complete the submission process.

  • Registered Student Organization: Chapter maintains registered student organization status with university.
